In Case of Injury

Filling out the accident form (for more serious injuries) makes it easier to file a damage report later.

All FC Futura Juniors participants must have a valid insurance policy that covers football activities. Insurance is mandatory for all children and youth, regardless of whether the player participates in youth or adult matches.

In case of an accident, instructions for those covered by the player pass insurance can be found via the link below. If the player has their own sports or accident insurance, please follow the guidelines provided by their insurance company.

Instructions for Players Covered by the Finnish FA Player Pass Insurance in Case of Injury.
If an injury occurs, please follow the instructions below.

Submit a damage report using the link provided below. After submitting the report, you will receive a claim number via email.
(Note: Be sure to check your spam folder.)

• Submit a damage report using the link
• Seek medical attention within 2 weeks of the injury
• Be prepared to pay the first private doctor’s fee yourself
• Medical clinics may invoice follow-up visits and examinations directly from the insurance company when you present your claim number and KELA card. A payment commitment may be granted for surgery and physiotherapy
